Specimen Requirements
Container/Tube: Slide(s)Specimen Volume: Entire specimen
Collection Instructions: Submit hematoxylin-and-eosin slides, a copy of the preliminary report, and a brief explanatory note. When appropriate, please include special stained slides, unstained slides, or paraffin blocks. Special staining methods require paraffin blocks for most stains. A few stains require frozen tissue.
Note: 1. Label container with patient’s legal name, date of birth, collection date, clinician, and specimen source.
2. Specimen source, clinical history/diagnosis, and physician’s name are required.

This request will be processed as a consultation. An interpretation will be provided. Appropriate stain(s) will be performed and charged separately.
